Folak Healthcare LLC is a certified DDD Adult Social Day Program where adults with disabilities are met, 21 and older across all counties
We saw families searching for a program that could meet complex behavioral and physical needs without compromising on warmth or community connection, and we built Folak HealthCare to be that place. Located at 1303 NJ 27 in Franklin Township, Somerset, NJ, we serve individuals aged 21 and older across Burlington, Mercer, Middlesex, Monmouth, Somerset, Sussex, and Union counties
Folak HealthCare, LLC is a day program for individuals aged 21 and older.
We provide programming that helps individuals maintain and build independence through routine, structured activities — both at our facility and out in the community across all counties.
We are passionately dedicated to empowering adults with intellectual and developmental disabilities by promoting exceptional achievement and fostering a strong sense of independence. Every facet of our services is centered on the distinct abilities and requirements of each person we serve.
At Folak HealthCare, we believe personalized support is the key to success. We offer high-quality programming, meaningful skill-building, and vibrant community engagement designed to be accessible and responsive at every stage of a person’s journey. We strive to create an environment where everyone feels valued, empowered, and capable of achieving their personal goals.
We also welcome individuals who may have had difficulty finding a day program within the CCP/SP system that can meet their behavioral and specialized needs — including individuals with limited physical/aggressive behaviors — because we believe everyone deserves a program built around them.
A welcoming environment where every participant feels secure, with on-site nursing staff and BCBA support available as needed.
Activities designed around real interests, hobbies, and friendships — not a one-size-fits-all schedule.
Caregivers who treat every person with dignity and respect, at a supportive 1:6 group ratio (smaller groups available as needed).
Real inclusion through outings, volunteering, shopping trips, and connection — at least one community activity daily, weather permitting.
